I stumbled upon this module because flag_abuse is broken and I needed this kind of functionality. It works well, integrates very well with flags and has some nice features like token support. Some ajax magic would be nice because I don't like that the note part has to be filled on a separate page. The docs are basic.
This module is very easy to install and configure. It took me about 5 minutes. I could set up a rule using the flag_note token as well! For me a good substitute for flag abuse module and flag content module.
It is also possibe to use the CAPTCHA module.
